Health Risk Assessments

July 31 - August 12

Health Risk Assessments (HRAs) are held annually for Grande Cheese Associates, along with their spouses or domestic partners. The HRA is a great opportunity to learn about your personal health, maintain your current health, and learn how to manage any health risks that might be present. 

The HRA consists of a biometric screen, online health questionnaire, and scorecard - details below.

To continue to encourage all Associates, Spouses and Domestic Partners to participate we will again offer a $650 annual discount on the medical premium if Associates participate and achieve 100 points on their scorecard. Beginning in 2018 we will offer an additional medical premium discount if spouses or domestic partners participate and achieve 100 points on their scorecard.

In order to receive the discount for 2018, you must participate in the HRA this year. The discount will be announced during Open Enrollment communication this fall.

Biometric Screen

The biometric screen will be conducted in private and results will be confidential. The screen will be performed by our Grande Health and Wellness staff or an Agnesian Work and Wellness health screener and will include the following:

  • Height, weight, and body mass index (BMI) measurements
  • Blood pressure reading
  • 12-hour fasting blood draw to measure cholesterol (Total, LDL, and HDL), glucose (blood sugar), cotinine (nicotine), and triglycerides (blood fats), and an optional PSA test for men over age 50

For the most accurate results, you must fast for at least 12 hours before your blood is drawn. If you don't fast the appropriate amount of time, your blood results will not be accurate. Drink plenty of water during your fasting time - it will help your stomach feel fuller, and keep you hydrated making the blood draw easier. There will be small snacks provided after your HRA appointment. Feel free to bring any extra food or drink you may need.

Online Health Questionnaire

Once your biometric screen is complete you will have to complete the online health questionnaire through Agnesian's Know and Go site (www.knowandgo.org).

If this is your first time completing the HRA, you will need to set-up your account. To do so, follow these steps:

1. Go to www.knowandgo.org

2. Select "Agnesian Wellness Business Partners"

3. Under First-Time Users: Register Here; select "Click Here to sign up"

4. Next, create a username and password. Be sure to hang on to this information as you will need it to log on view your scorecard and biometric results later.

5. When filling out your member information, use your full first name and last name with birthdate.

Previous HRA participants will already have an account. To access your account:

1. Go to www.knowandgo.org

2. Select "Agnesian Wellness Business Partners"

3. Log in using your username and password

To take your online health assessment:

1. Click on the Health Assessment button on the left side of the page to begin. Be sure to complete the whole assessment, partially completed assessments will not be counted.

Biometric Scorecard

As we continue in our efforts to build a healthy culture and encourage positive lifestyle behaviors among Associates and their families we will include the biometric scorecard as part of the HRA. Each year our goal is to continue to shift Grande's qualifying ranges closer to that of the optimal ranges set-forth by AMA/CDC.

Below you will see the updated 2017 HRA scorecard, new this year we have adjusted the qualifying range for fasting glucose and BMI. Your glucose numbers are a strong indicator of your risk for developing type II diabetes. If elevated glucose numbers are found early you can make lifestyle changes such as diet and exercise to help bring those numbers within normal range. If left untreated, you are at greater risk for developing type II diabetes and will be required to take costly medications in order to manage the disease. The small shift in BMI from 30.9 to 30 would indicate a weight loss of 2-3lbs, which can be significant enough to improve other areas of your health such as glucose, blood pressure, and cholesterol levels.

We are responsible for taking charge of our health. 7 in 10 Americans have at least one chronic disease and these types of conditions can be prevented through practicing healthy lifestyle behaviors like consuming a healthy diet, being physically active and maintain a healthy weight, not using tobacco products, limiting the consumption of alcohol, and practicing stress management techniques.  

We know that behavioral lifestyle changes are the hardest to make and to maintain, so we offer health coaching to our Associates, their spouses and domestic partners as a way to help you make those changes. Our health coaches will address your health concerns, help you develop goals, and work with you in achieving those goals and an overall healthier lifestyle.

For those that do not reach 100 points on the scorecard, you have the opportunity to earn those points back through participating in four health coaching sessions throughout the year. You have the choice of completing them in-person through the Grande Health and Wellness Centers or telephonically with an Agnesian Health Coach. You will receive more information after your the HRAs are completed in regards to the process of signing up for health coaching and completing your four sessions.
